How Can You Register?

- Each attendee must submit a separate registration form.
Combined payments are accepted.
- Payment must be included at the time of registration.
- Registration forms and payment should be mailed. Faxes or emails are not accepted.
- Mail registration must be postmarked by August 31, 2009 to be eligible for the early
bird rate.
- On-site registration will be available. Cost is $300 per person.
- Cancellations must be made in writing. No cancellations after August 31, 2009.
