Each attendee must submit a separate registration form. Combined payments are accepted.
Payment must be included at the time of registration.
Registration forms and payment should be mailed.
Faxes or emails are not accepted.
Mail registration must be postmarked by August 29, 2008 to be eligible for the early bird rate.
On-site registration will be available. Cost is $300 per person.
Cancellations must be made in writing. No cancellations after September 5, 2008. All cancellations should be sent to Attn: Mary Miller.
