Driver History Abstract


The MVC maintains your driver history in a document known as a driver history abstract.  Learn how to read a driver history record abstract by downloading a helpful brochure - Understanding your driver abstract - including a sample abstract, definitions and permitted users.

You have three options to obtain a certified, five-year driver history abstract:  online, in person at a motor vehicle agency, or through the mail.

Online

Customers are encouraged to use the online abstract request system. The MVC has waived the online convenience fee to process this transaction; however, there is a $15 fee for the record search.

In person

Customers may visit a motor vehicle agency.  You may complete the Application for Driver History Abstract (Form DO-21) prior to your visit, or obtain a copy at the agency.  A $15 fee will apply.  The MVC accepts American Express® card, Visa® card, MasterCard®, checks, cash and money orders.

Mail

Customers who submit a request through the mail must include the following:

  • A completed and signed Application for Driver History Abstract (Form DO-21); choose Certified Complete Record or Certified 5 Year Record within Section B; explain in detail the reason for your request;
  • A photocopy of your driver’s license; and
  • A check or money order for $15 payable to NJMVC.
  • If you are requesting a record that is not your own, you must complete Section C of the form.
  • Submit the package to:

Request driver history abstract and certified documents by mail

If you are requesting both certified copies of supporting documents (Order of Suspension; Schedule of Suspension; Restoration Notice, Mailing List; Summons; and Accident Report) and a driver history abstract:

  • Follow steps described above for abstract purchase.
  • On the same form (Form DO-21), Certified Complete Record or Certified 5 Year Record, and select the document type(s) within Section B and fill in the date of each supporting document; and explain the reason for your request.
  • Include a separate check or money order payable to NJMVC for the total cost of each request type; one check for the abstract total and one check for the supporting document total ($15 each).
  • For example, submit two checks - $15 check for an abstract and a $30 check for supporting documents - when ordering an abstract, certified order of suspension and a copy of a mailing list.

Request certified documents without a driver history abstract by mail

If you are requesting only certified copies of supporting documents:

  • Follow steps describe above for abstract purchase; except, select the  supporting document type(s) in Section B and fill in the date of each supporting document and explain the reason for your request.
  • Select the permitted use for your request and mark the number on the form (Form DO-21)
  • If you are requesting a record that is not your own, you must complete Section C of the form (Form DO-21).
  • Include a photocopy of your driver’s license
  • Include a check or money order payable to NJMVC (amount will vary depending on the number of records requested).

Important note: Both pages 1 and 2 must be completed and submitted for your request to be considered. You may print the form on both sides of a single sheet or print and attach the two separate sheets. A separate request is required for each abstract.

The driver history abstract and/or supporting documents requested will be mailed to you within seven business days of receipt of your completed form and payment.
